Your First Visit

Please assist us by providing the following information at, or before, the time of your appointment:

  1. Most recent x-rays from your previous dentist. Even x-rays that are two or three years old are beneficial in providing a “benchmark” of your oral health.
  2. A list of all medications you are presently taking, with dosages.
  3. If you have dental insurance, please bring the necessary information including the insurance card (if one was issued) and the social security number, date of birth and the employer of the subscriber (the person who has the insurance coverage). This will save time and allow us to accurately process any claims.
  4. Please consider printing the patient registration forms from this website and filling them out in advance.

A parent or guardian must accompany all patients under the age of 18 years of age to the initial appointment.

Your initial visit will include:

  1. Your medical history
  2. Your dental history
  3. Digital x-rays, if needed
  4. A thorough oral and soft tissue evaluation
  5. A discussion of your current concerns
  6. A dental prophylaxis, or teeth cleaning
  7. A detailed explanation of any findings
  8. Your treatment options
  9. Addressing your questions
  10. Oral care instructions
  11. Scheduling a date and time for your next visit

IMPORTANT: Please alert the office prior to treatment if you have a medical condition that may require pre-medication with antibiotics (i.e. joint replacements, artificial heart valves, history of rheumatic fever, certain heart murmurs, etc.) or if you are on aspirin or anticoagulant (blood-thinning) therapy.

NOTE: Due to federal HIPAA privacy laws we are unable to request x-rays from your previous dentist without a signed medical records release form. Please contact your previous provider and ask for their procedures for releasing dental records.
